Prior Lake High School and Middle Schools are required to follow the Minnesota State High School League (MSHSL) rules that require sports physicals for all MSHSL sanctioned sports.
The sports physical must clear the student for participation in sports, be dated within three years, and remain current throughout their sport/activity season. Without this up-to-date clearance physical, you will not be able to register your student for athletics.
If you are unsure if your student has a physical on file, please contact Heather Durling at hdurling@plsas.org or Lauren Albrecht at lalbrecht@plsas.org.
Sports Physicals may be submitted by email, FAX or in person. Please scan and email your student's sports physical to one of the staff listed above in the PLHS Activities office, or you can FAX it (952) 226-8689, or drop it off in-person at the Activities Office during summer office hours (9AM-3PM).
